Instructions for On-Line Session Evaluations

You will be required to put in your badge number. This will bring up your name and contact information.


Locating Your Badge Number
Lower Right Hand Corner.
Enter in Badge Number
Once you have entered in your number, click on Log In.


You are ready to begin. After you have completed an Evaluation Form, a large check mark will indicate this next to that Evaluation. You will see “Edit” and the number of Credit Hours. To start a new Evaluation, click on Evaluate.



Filling in the Evaluation Form is very simple. Just place the cursor in the selected circle and click. Before you begin, you need to select the kind of credit you want, either CME for physicians or contact hours of contact hours for allied health professionals.



When you have completed the form and are ready to submit your evaluation, click Submit Evaluation. You will be returned to the main menu to select your next evaluation or to print your transcript.
